Please have your team captain fill out the enclosed form with the four names of the individuals on the team, and send us 1 check only for your team registration. We have 50 carts at Lochland Country Club, so carts will be allocated on a first-come, first-served basis. Please provide all golfers 9 Hole Handicaps
Telephone entries ARE NOT ACCEPTED. We must have this form and payment in hand. Entry deadline is Tuesday, August 22rd.
ENTIRE team payment required for registration to be accepted. NO CASH PLEASE.
This event is limited to the first 124 individuals to sign up. Carts will be allocated on a first-come, first-served basis to the first 100 golfers.
